Findings
On or about August 9, 2020, Alleged Victim (AV) suffered from a fall when he/she was left unattended in the bathroom. An investigation determined that facility staff did not follow AV's care plan at time of AV's fall. The facility's failure to follow AV's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
