Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV's care plan did not address how to ensure AV's safety while having his/her legs elevated in his/her wheelchair. AV was in his/her wheelchair when staff elevated AV's legs. AV stretched his/her legs, causing the wheelchair to fall over backwards, resulting in a hematoma to AV's head. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
