- Alleged Victim (AV) had a known history of falls. AV had approximately eighteen (18) falls from February 19, to August 1, 2020. On or about August 11, 2020, AV was sitting on the toilet with staff present when AV fell to the side. AV was sent to the hospital and return the same day with scalp laceration, skin tear of right upper extremity and subarachnoid hemorrhage following the fall. AV had sustained sixteen (16) falls before the care plan was revised to reflect any interventions. Two (2) of the sixteen (16) falls occurred in the bathroom and AV’s revised care plan did not address falls or care during toileting. The facility failed to care plan related to AV’s known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
