Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to provide AV with the basic care and supervision needed to keep AV safe which resulted in AV falling and obtaining a fractured rib and a punctured lung. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
