Findings
The facility failed to follow Witness 1's care plan to monitor him/her during times of agitation. The failure resulted in a physical altercation with the Alleged Victim, who was transferred to the hospital for stitches to the head due to the altercation, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
