The facility failed to appropriately care plan and monitor Witness 1 (W1) according to his/her known behavior. The failure resulted in the Alleged Victim (AV) trying to pass W1, who reached out and scratched AV causing unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
