Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to implement reasonable interventions aimed at mitigating or preventing AV's frequent falls which resulted in AV experiencing six documented falls between July 12, 2020 and August 26, 2020 with the fall on August 26, 2020 resulting in a head injury. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
