Findings
On or about August 23, 2020, it was observed by the Alleged Victim's (AV) spouse that AV had bruising on his/her arms and scratches on his/her hips and buttocks. These wounds are consistent with staff pulling AV's arms to get him/her from bed and the scratches from staff having long nails despite the facility policy against long nails. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
