The facility failed to provide appropriate services according to the Alleged Victim’s (AV) needs. The failure resulted in AV being found in urine soaked clothes twice on August 31, 2020 and again on September 2, 2020 in the same clothes, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
