Findings
The facility failed to provide a safe environment for Alleged Victim (AV) by failing to implement reasonable, person centered interventions aimed at preventing or mitigating AV's falls which resulted in four falls between August 11, 2020 and August 31, 2020. AV was transported to the hospital and diagnosed with a fracture due to fall on August 27, 2020. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
