Findings
The facility and the Alleged Perpetrator 2 (AP2) failed to provide appropriate services according to the Alleged Victim’s (AV) needs. The failure resulted in AV being left on the toilet for approximately 45 minutes causing unreasonable discomfort and a loss of personal dignity, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
