- Alleged Victim (AV) moved into the facility on August 25, 2020. On or about September 8, 2020, in the morning staff discovered that AV was not in the facility. AV was found at his/her previous home, approximately 22 miles away from the facility. The facility was not able to determine exactly when AV left the facility however staff did not see AV from at least 21:30 on September 7, 2020 through 7:00 am September 8, 2020. Prior to this incident staff report AV exhibited exit seeking behaviors. The facility failed to care plan according to AV's exit seeking behavior putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
