The facility failed to provide a safe environment according to the Alleged victim's (AV) needs. On or about September 5, 2020, AV was attempting to transfer without assistance when an untrained staff attempted to assist, AV without a gait belt, as directed in the care plan; The failure placed AV at risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
