Findings
The facility failed to provide the Alleged Victim (AV) with prescribed medications from April 2020 until August 2020, which placed AV ask risk for serious harm. The facility also failed to notify AV's Physician regarding AV not receiving medications as ordered. AV was seen and the hospital and it was determined that AV had an irregular heartbeat and high blood pressure. The facility's failures are violation of residents rights, are considered neglect of care and constitute abuse.
