Findings
On or about September 12, 2020, Alleged Victim (AV) suffered from an unwitnessed fall in his/her apartment. An investigation determined that facility staff assisted AV to his/her chair after the fall; however, 911 was not called and AV was not medically evaluated due to there not being a clinical staff on duty at time of AV's fall. AV showed a significant change of condition after the fall and facility did not implement any safety measures to make sure AV was safe. AV was sent to the hospital for treatment on September 13, 2020. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care, and constitutes abuse.
