The facility failed to ensure a safe environment and staff support regarding known behaviors Witness #1 (W1) hit the Alleged Victim (AV) on the head with a vase, causing bruises to his/her head and a skin tear on the left elbow. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
