Findings
On or about September 10, 2020, the Alleged Victim (AV) was evacuated from the facility due to a fire. Upon evacuation, the facility failed to bring AV's CPAP machine, which AV uses nightly while sleeping to ensure oxygen stability. On or about September 13, 2020, AV was transported to the hospital and found to have a low oxygen saturation level. The facility failed to provide service to AV by not having his/her CPAP machine available. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
