Findings
The facility failed to appropriately care plan and provide appropriate services according to the Alleged Victim's (AV) needs, related to number of falls with lack of appropriate interventions. Based on the investigation, AV experienced multiple falls, between August 1, 2020, and September 26, 2020, which resulted in unreasonable discomfort and some skin tears. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
