Findings
The facility failed to follow the Alleged Victim’s (AV) care plan to lock his/her door to keep another resident from entering his/her room. The failure resulted in another resident entering AV's room to take his/her wheel chair, when AV went to stop them, he/she experienced a fall, causing him/her to be transferred and admitted to the hospital with a brain bleed, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
