Findings
The Alleged Perpetrator 2 (AP2) failed to provide appropriate services when the Alleged Victim (AV) asked for a non-skid mat to be provided while bathing. The failure resulted in AV slipping and experiencing a fall with injury. AP2's actions are considered neglect of care and constitutes abuse. The facility failed to rule out injury when AV complained of pain for approximately 5 days, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
