Findings
On or about August 22, 2020, the Alleged Victim (AV) was left in his/her wheelchair all night without any staff checking on him/her all night. Morning staff found AV in his/her wheelchair and AV stated he/she had been in his/her chair all night, leaving AV at a risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
