- On or about mid January 2020, documentation noted that the Alleged Victim (AV) had a bruise on his/her left forearm. AV reported to staff that the bruise happened from staff helping him/her out of bed or it was from the blood pressure cuff, however, the bruising was on his/her forearm, which is not where the cuff would be used. Alleged Perpetrator #2 (AP2, unknown) may have been responsible for the abuse, however, facility documentation does not have enough information to determine who AP2 would be and whether or not bruising was caused by a staff member, therefore, the finding for AP2 is inconclusive. The facility failed to report bruises of unknown origin to Adult Protective Services (APS) and failed to complete their own investigation thoroughly to rule out abuse. This inaction placed AV at risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
