- Alleged Victim (AV) had it noted in his/her care plan that he/she requires assistance with bathing, often refuses bathing, and goes out of the facility to get his/her hair washed. Due to COVID restrictions AV was unable to go out of the facility to get his/her hair washed. As of October 8, 2020, AV had not gotten his/her hair washed at the facility and it was unknown when AV's hair was last washed. The facility did not update AV's care plan to address AV being unable to go out of the facility to get his/her hair washed and did not address AV's refusal of bathing. Sometime in September 2020, AV could not locate two necklaces and some hair clips. After AV reported the missing items to the facility, the facility did not assist AV to go through his/her property to locate the items. The facility failed to plan care around AV's refusals of bathing and being unable to go out into the community to get his/her hair washed, and the facility failed to provide a safe environment resulting in AV losing his/her property, which is a violation of resident rights, is neglect of care and constitutes abuse.
