- Alleged Victim (AV) requires facility assistance with medication administration and has a known history of suicidal ideation. On or about October 26, 2020, AV went to the hospital and disclosed that he/she had been stashing his/her medication and had a suicide plan. AV continued to express suicidal ideation after being discharged back to the facility from the hospital. AV's healthcare provider contacted the facility and informed the facility that they needed to ensure AV was swallowing all of his/her medication. A visiting nurse found a pill cup containing pills sitting in AV's room. The facility failed to care plan and ensure AV was not stashing his/her medications putting AV at risk for serious harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
