Findings
The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV) received all medications that he/she was supposed to receive. Alleged Perpetrator #2 (AP2) falsified medication records at the facility to show medication had been given to AV as well as other residents when in fact he/she had not given the medication. The medication was either taken home or thrown in the garbage by AP2 rather than given to the AV as ordered. AP2's actions are considered neglect of care and constitutes financial abuse. The facility's failures to provide a safe medication administration system and ensure staff oversight are a violation of resident rights, are considered neglect of care and constitute abuse.
