Findings
On or about August 22, 2020, the facility recieved a medication order for Alleged Victim (AV) increasing one of his/her medications. On or about October 6, 2020, an error was discovered that the medication change was not implemented. AV did not suffer a negative outcome due to the medication error. The facility failed to provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
