- On or about July 14, 2020, Alleged Victim (AV) and Witness 1 (W1) were involved in an injury altercation. An investigation determined that W1 has a history of altercations and aggressive behaviors and was involved in approximately eight (8) resident to resident altercations since July 8, 2020. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
