Findings
Alleged Victim (AV) is a resident of the facility and relies on facility staff to meet his/her basic needs. In February 2020, AV weighed 205 pounds, in September 2020, AV weighed 182 pounds. AV had an 11% decrease in weight over a period of seven months. The facility failed to address AV's change of condition and implement interventions, which is a violation of resident rights, is neglect of care and constitutes abuse.
