Findings
The facility failed to provide a safe medication administration system for the Alleged Victim (AV) when they failed to check AV's blood sugar levels prior to administering insulin to AV. This action placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
