- Alleged Victim (AV) relies on the facility for his/her care. On or about September 3, 2020, AV’s medication change was incorrectly submitted which caused AV to be overmedicated and lead to AV sustaining several physical injuries due to unwitnessed falls. AV was found on the floor fourteen (14) times between September 1 through October 14, 2020. The facility failed to assure AV was safe, which is a violation of resident rights, is neglect of care and constitutes abuse.
