- The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV) received his/her medication as ordered to control his/her seizures. On or about October 17, 2020, AV was transported to the hospital for treatment and it was discovered that AV's levels of the medication were very low, indicating that AV was not receiving medication as ordered. On November 13, 2020, AV again went to the hospital and again had very low levels and was in serious condition. Facility medication administration records were unclear as to whether medications were administered, refused or missed, or if staff took additional steps after refusal of medications. Staff were not properly trained in medication administration and what to do in most situations. The facility's failure are a violation of resident rights, are considered neglect of care and constitute abuse.
