- On or about September 26, 2020, the Alleged Victim (AV) was given his/her "as needed" medication on three occasions by three different people, placing AV at risk for harm. Alleged Perpetrator #2 (AP2) gave AV his/her as needed medication after it had already been given by another employee. Alleged Perpetrator #3 (AP3) did the same, and also gave AV the same medication. These administrations of medication all happened within 3 hours, prescription dictates AV should only have this medication 1 time every 8 hours. AP2 and AP3 failed to follow protocol of checking the Medication Administration System to ensure AV's medications were not already given. AP2's and AP3's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to provide a safe medication administration system is a violation of resident rights, is considered neglect of care and constitutes abuse.
