Findings
The facility and Alleged Perpetrator #2 (AP2) failed to provide a safe medication and treatment system to ensure physician orders were followed for the Alleged Victim (AV) to be tested timely for a urinary tract infection (UTI). AV was to be tested for a UTI and it fell through the cracks and AV's sample was not sent out timely. AV suffered unnecessary pain and suffering through having a UTI without treatment. The facility and AP2's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
