Findings
The Alleged Victim (AV) was care planned to be checked four times per shift to assist with hygiene, changing soiled clothes/linens. AV was found incontinent and h/her clothes, bed, and couch were observed to be soaked in urine six times between September 2021- October 2021. The facility failed to follow AV’S care plan, the failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
