Findings
On or about October 17,2020, Alleged Victim (AV) and Witness 1 (W1) had a resident-to-resident altercation in which W1 slapped AV on his/her face leaving red marks. W1 has known aggressive behaviors towards staff and residents. The facility failed to provide a safe environment for AV by failing to protect AV from W1's aggressive behaviors. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
