Findings
According to documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim's (AV) care plan to ensure she/he is wearing non-skid socks to prevent fall and injury. The failure resulted in AV being found on the floor without proper socks causing unreasonable discomfort and risk of serious injury, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failed to protect AV from neglect, which is a violation of Oregon Administrative Rules.
