Findings
Witness 1 (W1) had a known history of experiencing psychosis and hallucinations with aggressive behavior. On or about October 27, 2020, W1 was experiencing psychosis and physically attacked Alleged Victim (AV), punching AV several times resulting in AV having discoloration to his/her face. The facility failed to care plan for W1's known behavior, which is a violation of resident rights, is neglect of care and constitutes abuse.
