Findings
Alleged Perpetrator 2 (AP2) failed to provide the basic care and services necessary to keep Alleged Victim (AV) safe when AP2 gave AV another resident's medication which caused risk of harm to AV. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules.
