Findings
The facility failed to appropriately care plan for the Alleged Victim (AV) regarding his/her risk of falls. AV was found in his/her room with an abrasion above his/her eye and an abrasion to his/her left arm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
