Findings
Alleged Victim (AV) has known exit seeking behaviors. On or about September 13th, 25th and October 10, 2021, AV exhibited exit seeking behaviors. on or about October 19, 2021, AV eloped from the facility. The facility failed to care plan according to AV’s known behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
