Findings
On or about November 1, 2020, Alleged Victim (AV) suffered an unwitnessed fall that resulted in AV getting a knee abrasion. An investigation determined that AV suffered from seven (7) falls since August 2020. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
