Findings
Alleged Victim (AV) and Witness 1 (W1) are care planned to be kept separated due to previous physical altercations. On or about November 6, 2020, W1 and AV got into an altercation resulting in AV sustaining skin injuries. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
