Findings
Witness 1 (W1) was care planned to be observed for aggressive words/behaviors. On or about November 6, 2020, W1 was showing aggressive behaviors, W1 came up to Alleged Victim (AV) and pulled him/her to the ground. The facility failed to follow care plan related to W1’s known behavior of aggression, which is a violation of resident rights, is neglect of care and constitutes abuse.
