Findings
On or about October 5, 2020, the facility failed to provide the basic care or services for Alleged Victim (AV) by failing to prevent AV from eloping from the facility. The facility's failure to provide a safe environment for AV is a violation of resident rights, is considered neglect of care which constitutes abuse.
