Findings
The facility failed to implement interventions and appropriately care plan related to Witness 1's (W1) known behaviors resulting in W1 transferring Alleged Victim (AV) from his/her recliner without assistance from staff which resulted in AV suffering shoulder and back pain. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
