Findings
On or about November 11, 2020, the Alleged Victim (AV) exited the facility through a door without an alarm, unknown to staff. AV came back into the facility on his/her own and was unharmed, however, AV was placed at risk of harm by being able to exit the facility without knowledge of staff. The facility failed to provide a safe environment, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
