- On or about September 8, 2020, the Alleged Victim (AV) reported his/her wallet missing. On or about August 27th or 28th, a staff discovered AV's wallet in the washing machine and gave the wallet to Alleged Perpetrator #2 (AP2) who laid the wallet in the medication room to dry it out. Alleged Perpetrator #3 (AP3) admitted to seeing the wallet in the medication room, but failed to follow facility policy to count and document the money and ensure that all items were returned to AV. On September 8, 2020, AP3 was told by other staff that AV's wallet and money were missing, and failed to complete an internal investigation or report the incident to Adult Protective Services (APS). On September 9, 2020, AP3 and Alleged Perpetrator #4 (AP4) determined that the cash in the wallet was taken by AP2, however, did not place AP2 on leave pending the full investigation and allowed AP2 to continue to work. AP4 failed to follow facility policy regarding investigations and directed staff not to communicate with APS, Police or others regarding the investigation. AP2's actions are considered neglect of care and constitute financial abuse. AP3 and AP4's actions are considered neglect of care and constitute abuse. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
