Findings
The facility failed to implement interventions and appropriately care plan related to Witness 1's (W1) known behaviors and aggression. The facility's failure resulted in W1 threatening the Alleged Victim's (AV) life causing emotional distress, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
