- Alleged Victim (AV) was care planned as independent with ambulation. On or about August 28th or 30th, 2020, AV began developing symptoms and the facility contacted AV's medical provider. August 31, 2020, service assessment states that AV's medical provider is to be faxed with any concerns. On or about September 5, 2020, AV tripped and fell and was sent to the emergency room with pain, and the facility noted that they will continue to encourage AV to use his/her walker. AV was diagnosed with a urinary tract infection while in the emergency room. On or about September 7, 2020, AV experienced approximately six (6) falls, some with injury. The facility failed to care plan appropriately and implement interventions to mitigate AV's risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
