Findings
On or about November 21, 2020, the Alleged Victim (AV) and Witness #1 (W1) had a non-injury altercation. The facility failed to provide a safe environment for the AV, resulting in AV being hit in the stomach by W1 and W1 being hit in the face by AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
